Nurses are essential players in the community based on the services they provide. Their interactions with the patients allow them to document some of the best recommendations to counter the current health challenges influencing patient health outcomes. This project has selected my family member who has been diagnosed with chronic hypertension. This condition has been recognized as one of the most common illnesses affecting the population. This condition can be managed through various interventions. One of these interventions is lifestyle changes. Lifestyle changes such as diet modifications, increased physical activity, and smoking cessation are evidence-based solutions for managing the condition. However, a lack of proper follow-up may undermine the effectiveness of the nursing interventions. It is worth noting that out of four, only one person can control their blood pressure. Therefore, this problem is rampant in the country, affecting the quality of life of many people. Furthermore, the condition is associated with numerous risk factors such as obesity and the adopted lifestyles. This project will work with a family member who has been diagnosed with high blood pressure. My interactions with the patient were based on analysis and documentation of the medications and interventions initially adopted to foster better health outcomes. The patient is a 45-year-old male who has been diagnosed with hypertension. His lifestyle revolves around long sitting hours. Additionally, it is believed that the age factor is a major contributing aspect in this trend leading to reduced connection with better health outcomes. The condition has been witnessed for over five years, with the patient reporting to various clinical centers. The provided interventions have provided little to no effects on handling this condition. Therefore, there is a need to develop the ideal approach for handling the condition using home-based remedies and clinical interventions. These incidents reflect the differences in lifestyles where many people who work in highly demanding environments are likely to suffer this condition. The problem is relevant to the client because most of the incidents occur based on aging. The patient is 45 years old. This problem is essential to my practice since it provides essential approaches for understanding some of the chronic illnesses affecting the population and the underlying factors undermining the recovery process. The American Nursing Association provides a foundation for the adopted practices within the care setting (Dolamo, 2018). State-based agencies such as the Texas Board of Nursing promote conditions and population wellness by licensing practitioners based on their capacities. This agency engages nurses in various aspects and practices that involve the care continuum, such as policy development. State-based boards provide guidelines influencing nursing actions and engagement in healthcare policy development and planning. This engagement empowers nurses in responding to the community demands while fostering better insight into the best strategies for accomplishing the ideal goals (Al Faouri, Elfaqieh & AbuAlRub, 2021). These guidelines provide nurses the ideal frameworks for responding to issues presented within the markets, such as illnesses.

From another dimension, the government plays a crucial role in the health sector by providing essential guidelines to nurses and other agencies. For example, the Affordable Care Act provides essential guidelines that shape nursing actions and relationships with their populations. This policy has been created to promote a paradigm shift towards a preventing care framework. This framework will promote care delivery to patients regardless of the prevailing economic weaknesses.

Through the policy, nurses and the respective practitioners can adopt measures for improved care delivery focusing on patient-centered interventions. Additionally, the government controls the nursing interventions and perceptions towards care delivery through the HIPAA (Health Insurance Portability and Accountability Act) policy. This policy governs patient data sharing frameworks and guidelines where nurses must maintain the ideal security and safety to promote reliability in services delivery. Furthermore, this policy promotes data sharing, which fosters nursing collaboration. Collaboration in hypertension treatment will promote a holistic approach in handling the witnessed challenges despite the presenting challenges.

Nurse leadership allows practitioners to implement reliable interventions in handling the increasing patient concerns. Adopting the ideal leadership frameworks and styles such as democratic improves workplace autonomy hence increasing the effectiveness of the adopted interventions. In addressing the problem mentioned, leadership must address collaboration and communication (Blackstone et al., 2017). The collaboration between nurses and patients is likely to produce better results since it is possible to discuss additional issues beyond the professional context. Nurses must understand the best communication and collaboration strategies to adopt in their practice to promote hypertension treatment and containment. Hypertension remains one of the most common health conditions affecting over 108 million people in the country. This condition requires adopting the ideal interventions for promoting operational awareness and lifestyle modifications to meet the current and projected health outcomes. Engaging nurses in the treatment processes empowers the community to respond to the increasing consumer expectations hence promoting operational efficiency. This project has selected hypertension because it can be managed through lifestyle changes. The effectiveness of the selected and recommended strategies depends on the communication and collaboration between nurses and patients. Leadership in the nursing field empowers practitioners to deliver evidence-based care.
